I might be far off here, but that boar head kind of looks like someone took greenstuff to a horse head. It would fit with the overall shape and the length of the neck, in my opinion. Could maybe the whole thing have been a conversion of some kind, with some sculpted parts to boot - ergo, not produced in any official fashion?

It's a Tim Prow sculpt. Tim also did the Goatmen war altar for Ye Alchemist (AKA Jason Lye of Hybrid Miniatures). That one was a Kickstarter project.
